Home Health Therapist Position

At Your Tailor Made Senior Service, we are seeking a dedicated and skilled Home Health Therapist to join our team. This role involves providing individualized therapy programs for patients in a home health environment.
About the Role:
- We are offering a competitive salary of $65,000 - $90,000 per year, depending on experience.
- This position is ideal for professionals who are passionate about delivering high-quality care and making a positive impact on patients' lives.
- You will work collaboratively with other healthcare professionals to develop and implement personalized treatment plans.
Key Responsibilities:
Designing and Implementing Treatment PlansYour primary responsibility will be to design and implement comprehensive treatment plans tailored to each patient's unique needs and goals. You will assess their functional abilities, identify areas for improvement, and create effective strategies for achieving optimal outcomes.
Providing Direct Patient CareAs a Home Health Therapist, you will provide direct patient care, evaluating their progress, and adjusting treatment plans as needed to ensure the best possible results. You will also communicate regularly with patients and their families to keep them informed and engaged throughout the treatment process.
Collaborating with Healthcare ProfessionalsYou will work closely with other healthcare professionals, including doctors, nurses, and therapists, to ensure seamless coordination of care and maximize patient outcomes. This may involve participating in interdisciplinary team meetings, sharing knowledge, and contributing to the development of treatment plans.
Maintaining Accurate RecordsYou will maintain accurate and up-to-date records of patient interactions, treatment plans, and progress. This includes documenting evaluations, treatment goals, and plans via our EMR system (Your Tailor Made Senior Services EMR System(s)-Finale Health). Regularly updating documentation and maintaining prescriptions and signatures is crucial.
Requirements:
- Bachelor's or Master's degree in Physical Therapy, Occupational Therapy, or Speech-Language Pathology.
- Current licensure or certification in your respective field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to adapt to changing situations and priorities.
