We are seeking a skilled Home Health Occupational Therapist to join our team in Menominee, Michigan. As an Occupational Therapist with LHC Group, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.

About the Role

The ideal candidate will be licensed as an Occupational Therapist in the state of practice, have current CPR certification, and a reliable vehicle for travel. You will be responsible for assessing and evaluating patient care needs, developing treatment plans, and implementing interventions to promote functional independence and quality of life.

Responsibilities
  • Evaluate patient's functional status, status of all body systems, and occupational therapy needs
  • Develop and implement individualized treatment plans
  • Conduct patient assessments and apply diagnostic and prognostic functional ability tests
  • Consult with physicians and other healthcare professionals to coordinate care
