MDS Coordinator Job Description

Mozaic Senior Life is seeking a skilled MDS Coordinator to join our team. As a key member of our healthcare team, you will be responsible for ensuring the highest quality of care for our residents.

Key Responsibilities:

  • Conduct initial admission, quarterly, and change of condition MDS assessments
  • Collaborate with Nurse managers and IDT to develop and implement care plans
  • Ensure all Medicare residents are skilled and meet the necessary criteria

Requirements:

  • Graduate of a school of professional nursing; BSN preferred
  • Strong clinical background in nursing, with experience in MDS and care planning
  • RN State of Connecticut, CPR certification, and IV-line management 3.0 certification
