Job OverviewThe RN Case Manager PRN coordinates and directs care specific to the needs of each patient, collaborating with the Rehab Program Director and Facility Case Management Director. You will promote consistent, positive patient interactions that advance unparalleled patient service.
Key Responsibilities:- Provide program orientation to patients/families/caregivers, including the case manager's role, Rehab philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient/family/caregiver education, promoting participation in team discussions related to plans, goals, and status through Family Conferences and other interactions.
- Ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences, facilitating involvement throughout the rehabilitation process.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient/family/caregiver.
- RN with current state licensure; BSN preferred.
- Three years' clinical acute care hospital nursing experience required.
- 2 years of case management experience.
- Certification in case management or utilization review preferred.
- InterQual experience preferred.
This per diem position offers flexibility and autonomy, with opportunities to work five shifts within each four-week schedule, including at least one weekend shift or four weekend shifts, and one major holiday and two minor holidays.
