About the Role

We are seeking a highly skilled and compassionate RN Case Manager to join our team at SSM Health. As a key member of our healthcare team, you will play a vital role in ensuring the seamless transition of patients from our hospital to their next level of care.

Key Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to ensure a safe and successful transition to their next level of care.
  • Collaborate with interdisciplinary teams to identify and address any barriers or gaps in care that may impact patient outcomes.
  • Participate in the development and implementation of care coordination strategies to improve patient satisfaction and reduce readmissions.
  • Provide education and support to patients and their families to promote self-management and independence.
  • Document and communicate patient information to ensure continuity of care.
Requirements
  • Graduate of an accredited school of nursing with a current RN license in good standing.
  • Three years of acute care experience with a focus on case management or care coordination.
  • Excellent communication and interpersonal skills with the ability to work effectively with patients, families, and healthcare teams.
  • Strong analytical and problem-solving skills with the ability to identify and address complex care coordination issues.
  • Ability to work in a fast-paced environment with multiple priorities and deadlines.
