Job Summary

We are seeking a skilled Nurse Care Manager to join our team in Southwest, Oregon. As a key member of our care management team, you will be responsible for providing comprehensive nursing care to patients receiving home infusion therapy.

Responsibilities
  1. Develop and implement individualized care plans for patients, including assessment of physical, psychosocial, economic, and environmental status.
  2. Conduct regular evaluations and updates of patient plans to ensure optimal outcomes and identify areas for improvement.
  3. Collaborate with interdisciplinary teams, including physicians, pharmacists, and other healthcare professionals, to provide seamless care coordination.
  4. Furnish necessary nursing services requiring specialized skill and judgment, including administering medications, monitoring vital signs, and providing education to patients and caregivers.
  5. Maintain accurate and complete clinical records, ensuring timely documentation and compliance with regulatory requirements.
  6. Communicate effectively with patients, families, and healthcare providers through excellent verbal and written communication skills.
  7. Participate in performance improvement initiatives to enhance quality and safety standards within the organization.
Requirements
  • Current unencumbered nursing license for the state of Oregon.
  • Minimum one year of experience in infusion nursing or related field.
  • Strong interpersonal and time management skills, with ability to work effectively in a fast-paced environment.
  • Proficiency in standard workplace software and information management applications.
