Nurse Specialist Venous Access Night Shift

We are seeking a skilled Registered Nurse to join our team as a Venous Access Nurse in a per diem capacity. This is an exciting opportunity to provide high-quality patient care and work collaboratively with a multidisciplinary team.
About the RoleThis role involves carrying out patient care responsibilities with a focus on decision-making related to the nursing process, including assessment, diagnosis, expected outcomes, planning, implementation, and evaluation. As a member of the healthcare team, you will formulate the initial plan of care with the provider physician or nurse practitioner and collaborate with them to implement a comprehensive patient-centered plan of care.
Responsibilities- You will demonstrate a solid foundation and understanding of organizational goals as outlined in the quality and safety performance improvement plan (QSPIP) and nurse-sensitive quality indicators, including unit-based quality measures such as HAPU, CAUTI, CLABSI, and Falls.
- You will collect, assess, and document patient health data.
- You will involve patients/families and the interdisciplinary team to identify a plan of care and outcomes that are realistic and measurable.
- You will prioritize care based on collected data.
- You will document assessments and reassessments in medical records.
- You will implement interventions in the coordination of patient care in a safe, timely, and appropriate manner, applying escalation processes as needed.
- You will consider the needs and behaviors of specific patient age groups and cultural groups in all patient care activities.
- You will document interventions according to hospital standards.
- You will educate patients/families regarding health status and established plans of care.
- You will determine learning needs of patients/families and implement educational plans, collaborating with the interdisciplinary team to modify expected outcomes.
- You will document learning outcomes of patients/families and revise education plans, utilizing EHR education tools as needed.
- You will evaluate patient progress towards expected outcomes, involving patients/families and the interdisciplinary team in the evaluation process, recognizing changes in patient condition, reporting, documenting, and evaluating the effectiveness of interventions.
- You will participate in and contribute to unit activities to provide patient care in a healing, humane, and caring environment.
- You will demonstrate understanding of and contribute to unit throughput efficiencies and metrics, including unit throughput measures such as DBN, door-to-triage, assignment-to-occupy, first-case-starts, and room-turnover.
- You will comply with unit-based and organizational professional standards, New York State Nurse Practice Act, and regulations of relevant outside agencies, such as the New York State Department of Health and The Joint Commission.
- You will exhibit appropriate judgment in delegated activities.
- You will demonstrate ability to differentiate between activities within the scope of professional nursing practice and those that may be delegated to ancillary staff.
- You will comply with annual education and mandatory/regulatory requirements, including learning transcripts and supportive documentation.
- You will serve as a support and resource for nursing staff and students.
- You will engage in self-assessment and peer review, utilizing resources to meet individual learning needs and promote professional development in clinical practice.
- You will adhere to patient care and nursing standards in care delivery and use clinical practice guidelines (CPG).
- You will attend unit practice councils (UPC) and utilize the practice council structure to critically appraise and influence practice, providing supporting evidence as needed.
- To qualify, you must have a Current Registered Professional Nurse license in New York.
- You must hold a Bachelor's degree in Nursing.
- You must possess Basic Life Support Certification through the American Heart Association.
- You must be fluent in written and spoken English language skills.
