PACT Vocational Specialist

The Department of Mental Health is seeking a dynamic and recovery-oriented professional to serve as the Brockton Multi Service Center PACT Vocational Specialist.
The selected candidate will be a member of a multidisciplinary team providing supportive counseling, psychotherapy, and wrap-around services to clients in the community.
The incumbent will provide psychoeducation, skill development, and social support to individuals with chronic mental illness, ensuring appropriate medical linkage and collaboration with community agencies.
The successful candidate will conduct ongoing assessments, treatment planning, and discharge planning, ensuring seamless transitions for clients.
Key Responsibilities:
- Provide wrap-around services for adult eligible clients, assessing their needs and developing supportive relationships.
- Serve on multidisciplinary teams, attending regular meetings and participating in discussions to ensure optimal care for clients.
- Develop, write, implement, evaluate, and revise overall treatment goals in conjunction with individual treatment teams.
- Assess vocational goals and assist clients in preparing for, seeking, and maintaining employment and/or education.
- Advocate for clients, acting as a liaison between the client and the community to ensure access to appropriate services.
- Support and teach clients in daily living tasks, assisting in entitlement acquisition and navigating the social service system.
- Provide individual and/or group readiness counseling, job skills training, benefits counseling, and referrals.
- Assess and respond to crisis situations, intervening with clinical treatment and contacting the on-call supervisor or Emergency Services Program as needed.
- Document all administrative and clinical record requirements, ensuring compliance with agency and Joint Commission standards.
Preferred Qualifications:
- Knowledge of employment and educational systems
- Familiarity with mental health diagnosis
