About Our Role

As a Medical Coordinator at the National Health Foundation, you will play a vital part in our Recuperative Care Program, providing exceptional post-hospital care to patients while creating a safe, healthy, and home-like environment.

Key Responsibilities:
  • Facilitate hospital discharge processes, ensuring seamless transitions for patients into our program.
  • Conduct medical assessments, create personalized plans of care, and maintain accurate guest charts via our existing CRM database.
  • Coordinate medical appointments, provide education on medication regimens, and promote preventative measures.
  • Collaborate with Intake teams to ensure eligible patients receive appropriate recuperative services and necessary equipment.
Requirements and Qualifications:
  • Current LVN License or Bachelor's degree in Nursing.
  • American Heart Association BLS (CPR/First Aid/AED) certification.
  • Experience working with homeless patients, bilingual skills in Spanish, and ability to work under stressful situations.
  • Excellent communication and social skills for building relationships with guests and colleagues.
