About the Job:

We are seeking a skilled Geriatric Medicine Nurse Practitioner to join our team for a part-time locum assignment in Chester County, PA. This opportunity will involve providing primary medical coverage to patients in collaboration with Primary Care Physicians as needed.

Key Responsibilities:

  • ">
  • Conduct initial, semi-annual, and annual assessments to develop appropriate plans of care for participants">
  • Participate in discharge planning for acute and long-term placement">
  • Evaluate and treat participants during acute illness">
  • Prescribe medications, therapies, and other treatments as necessary">
  • Collaborate with Medical Director and Clinic Supervisor to formulate clinical policies and standards of care">

Requirements:

">

To be successful in this role, you should have a Master's degree in Nursing and a minimum of 5 years of experience preferably in long-term care. Active PA licensure is also required. Additionally, you should hold BLS, CDS, DEA certifications and be board certified by ANCC or AANP. Certification as a Nurse Practitioner with prescriptive authority is also mandatory. Prior experience working with the frail or elderly population is strongly preferred.

Estimated Salary Range:

The estimated salary for this position is around $120,000 - $150,000 per year, depending on qualifications and experience. This estimate takes into account the level of responsibility, expertise required, and the location of the job in Chester County, PA.
