Job Opportunity: Home Health Nurse Practitioner

We are seeking a skilled Home Health Nurse Practitioner to join our team at Resilient Healthcare.

About the Role

The successful candidate will provide high-quality patient care in home health and hospital-at-home settings, working closely with our multidisciplinary team to deliver exceptional healthcare outcomes.

Key Responsibilities
  • Conduct initial patient assessments, develop personalized care plans, and implement evidence-based interventions to address chronic and acute conditions.
  • Perform laboratory testing, examinations, and treatments as required, collaborating with physicians as necessary.
  • Maintain accurate and comprehensive patient records, documenting statistics and research findings as needed.
  • Collaborate with staff to identify patient needs and develop plans to meet these needs, ensuring seamless continuity of care.
Requirements
  • Master's Degree in Science of Nursing or Physician Assistant Studies; Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas required.
  • Minimum 3 years' experience as an advanced practice provider, including experience in Home Healthcare.
  • Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants required.
  • American Red Cross or American Heart Association Basic Life Support Course (BLS or BLCLS) and Certification; American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification.
  • Satisfactory references from employers and/or professional peers.
  • Valid Driver's License and ability to travel throughout the area.
Compensation and Benefits

We offer a competitive salary range of $120,000 - $150,000 per annum, depending on experience, plus comprehensive benefits, including medical, dental, and vision insurance, retirement plan, and paid time off.
