Mass General Brigham is seeking a highly skilled Care Coordinator to support the long-term recovery of patients in Somerville, Massachusetts.

About the Role

This dynamic individual will work closely with our Case Management team to ensure seamless transitions from hospital to home, focusing on high-risk patients requiring ongoing care and support.

The ideal candidate will possess excellent communication skills, be able to work independently, and demonstrate a strong understanding of community resources and healthcare systems.

Key Responsibilities
  • Provide administrative support for discharge planning, including faxing, copying, and scanning information related to referrals, insurers, and PCP offices.
  • Assist patients with securing new PCPs prior to discharge and research DME providers.
  • Support patient referral processes to SNF, home care, and outpatient services, including entering clinical information into 4NEXT and managing communications with facilities.
  • Conduct insurance authorization and continued stay processes by making initial contact with payers and submitting requests for authorization.
  • Maintain databases for insurance-based processes, such as denials and appeals.
  • Contact patients post-discharge to complete follow-up phone calls and document interactions per department protocol.
