Gentiva Hospice is a leading provider of hospice care, and we are seeking a skilled Hospice Nurse Practitioner to join our team. As a member of our clinical staff, you will play a critical role in delivering high-quality patient care and supporting our mission to provide compassionate and personalized care to those in need.

Key Responsibilities:

  • Perform Face-to-Face (F2F) encounters and gather clinical information to support hospice diagnosis and prognosis
  • Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility
  • Conduct physical assessments and provide clinical findings to inform treatment plans
  • Collaborate with the Interdisciplinary Group (IDG) to develop and implement patient care plans

Requirements:

  • Master's or higher degree in Nursing
  • Current state license as a registered nurse and advanced practice nurse
  • Current national certification as a Nurse Practitioner (ANCC or AANP)
  • Collaborative agreement in place
