About the Role

We are seeking a skilled RN Liaison to join our team at Kaweah Health. As a key member of our healthcare services team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare providers.

Key Responsibilities
  • Respond to inquiries about healthcare services within one working day, providing professional and thorough program information.
  • Develop positive working relationships with physicians and staff involved in the referral and patient transfer process.
  • Perform site visits within 24 hours (M-F) to review medical information and assess patient needs, providing agency/facility choice and documenting choice and assessment on appropriate forms.
  • Communicate with Medical Director to make final determinations regarding patient admission, if applicable.
  • Establish availability of funds for required services and communicate with patient and administrative personnel throughout the process.
  • Communicate with patient, family, referral source, physician, and others regarding admission process and date, providing information on alternative options if admission is not possible.
  • Communicate with unit staff and leadership concerning admission date, bed needs, and special needs/equipment of patients to be admitted.
  • Represent and market healthcare programs to the community.
  • Maintain monthly statistics through referral logs.
Requirements
  • Current California RN license.
  • BLS certification.
  • Preferred: Completion of specialty certification, BSN, MSN, or currently enrolled in RN-BSN program.
  • Preferred: Two years of patient care experience in the appropriate service area.
