LNA II/Department Coordinator

As a Licensed Nursing Assistant II, you will perform a variety of direct patient care activities that fall within the scope of LNA practice as defined by the State Board of Nursing and Catholic Medical Center Nursing Organization policies and procedures. This includes utilizing basic knowledge of growth and development and awareness of age-appropriate care to provide care for patients across the life span.
- Assist the Registered Nurse, Licensed Practical Nurse, or physician to perform selected patient care activities or tasks as appropriate.
- Report all observations, symptoms, deviations from normal, and changes in patient's condition to the Registered Nurse and/or Licensed Practical Nurse.
- Perform patient rounds, respond to patient call bells promptly, and communicate with appropriate staff to meet patient needs.
- Provide basic support to patients and families in response to identified psychosocial needs and cultural and religious preferences.
- Perform or assist patients in the completion of activities of daily living, assist nursing staff with patient ambulation, transfers, range of motion, and repositioning as required.
- Anticipate patient needs and ask patients about need for toileting, fluid, nutrition, comfort measures, and maintain patient privacy and dignity.
- Participate in admitting patients by bringing patients to room and orienting patients to surroundings.
- Document in selected areas of the medical record, as appropriate, according to policies and procedures.
As a Department Coordinator, you will collaborate with the multidisciplinary team in the coordination of patient care services. This includes greeting visitors to the department or via telephone, ensuring customer service is a priority, and responding/referring visitors to appropriate patient rooms or department staff members.
- Coordinate all designated aspects of the admission and discharge process, which may include pre-registration, demographic and insurance verification, registration functions, assigns correct accommodation codes, and discharge disposition.
- Ensure that each patient has an order for code status, precautions, isolation, advanced directives, and all consults have been added as a care provider in Sunrise.
- Review and assist with completion of orders reconciliation on admissions, transfers, and discharges, print and report all unacknowledged orders at the beginning of each shift, and audit all overdue tasks and reports to the charge nurse.
- Change patient location and/or enter temporary location in Sunrise upon transfer to OR and clear current temporary location when patients return to the department from the OR/PACU.
- Monitor patient assistive devices tracking tool and advanced directives on admissions, transfers, and discharges for each patient on the unit.
- Assist with coordination of patient discharges.
- Complete charges in an accurate and timely fashion, audit and adjust patient charges, ensuring accuracy, to accounts as appropriate, and meet daily with case management to review.
- Monitor and/or update the following reports for compliance and complete data, including but not limited to patient profiles, indwelling urethral catheter report, central line days, pneumococcal and influenza vaccine, pain audits, and interdisciplinary nutrition referrals, and disseminate findings to nursing leadership and other staff as needed.

The ideal candidate will possess a high school diploma or equivalent, six months of LNA experience, and basic computer keyboarding and data entry skills. Additionally, the candidate must be able to utilize and navigate electronic clinical documentation systems to enter patient data.
- Current registration in the State of New Hampshire as a Licensed Nursing Assistant (LNA).
- BLS certified. If not BLS certified, must obtain BLS certification prior to completion of orientation, not to exceed 21 days from date of hire with Catholic Medical Center. Must maintain BLS throughout employment.
