About the Role

We are seeking a highly skilled Clinical Support Specialist to join our team at CN Guidance and Counseling Services. As a key member of our Youth ACT Team, you will play a vital role in delivering comprehensive and effective services to individuals and their families who require mental health support.

Responsibilities
  • Assist clients with transportation and representation at court hearings, ensuring they receive the necessary support during critical moments.
  • Collaborate with the team to complete referrals and linkage to community benefits, organizations, and institutions that align with our treatment plan goals, empowering clients to achieve positive outcomes.
  • Cultivate strong relationships with clients and their families, providing targeted service interventions to help them acquire essential skills and abilities for managing their health, improving family dynamics, and developing opportunities for prosocial activities.
  • Leverage your expertise to integrate educational and vocational goals and services with the tasks of all team members, ensuring seamless collaboration and maximizing the impact of our services.
  • Conduct assessments such as PHQ-9/a, suicidality evaluations, relapse prevention planning, and safety planning, fostering a safe and supportive environment for our clients.
  • Coordinate community integration trips for Youth ACT Recipients, promoting social connections and independence.
  • Contribute to Quality Improvement, Utilization Review, and Performance Improvement activities, driving data-driven decisions and enhancing our service provision.
  • Develop and implement case management and care coordination strategies, ensuring timely access to benefits through various government agencies and housing applications.
  • Maintain an understanding of concrete services, including Medicaid, Medicare, DSS, and SPOA housing, to provide informed guidance to clients.
  • Promote shared decision-making by facilitating the sharing of data with clients, empowering them to take ownership of their progress.
  • Participate in QI Training and refresher training as needed, demonstrating a commitment to excellence and continuous improvement.
  • Foster a culture of inclusion, respecting individual strengths, views, and experiences, and contribute to better business outcomes.
  • Adhere to all safety protocols and procedures, prioritizing a safe working environment for colleagues and clients alike.

What We Offer
  • A competitive salary range of $65,000 - $85,000 per annum, commensurate with experience.
  • An attractive and comprehensive benefits package, including medical, dental, vision, 403(b) retirement plan, long-term and short-term disability, legal plan, dependent care expense account, life insurance, pet insurance, HRA, and training programs.
