Job Overview

We are seeking a highly skilled Medical Service Provider to join our team at Health Care For the Homeless Inc. As a key member of our multidisciplinary team, you will be responsible for providing medical services to individuals and families experiencing homelessness.

About the Role

  • You will work collaboratively with a team of healthcare professionals to assess, diagnose, and treat patients in both clinic and community-based settings.
  • Effective communication and interpersonal skills are essential for building strong relationships with patients, colleagues, and other stakeholders.

Key Responsibilities

  • Assessment, diagnosis, and treatment of patients in accordance with established standards of care and evidence-based guidelines.
  • Providing quality chronic, preventive, acute, and outreach services that meet the unique needs of each patient.
  • Maintaining accurate and timely documentation of patient services in electronic medical records.
  • Collaborating with interdisciplinary teams to develop and implement integrated treatment plans.
  • Communicating effectively with patients, colleagues, and other stakeholders to ensure seamless care delivery.

Requirements

  • A Master's or Doctorate Degree in Nursing as an adult or family nurse practitioner.
  • Possession of current licensure as a registered nurse practitioner in Maryland.
  • CDS and DEA licenses, BLS CPR certification.
  • At least 3 years of direct clinical care experience in primary care, preferably in a multidisciplinary setting.

Working Conditions and Physical Requirements

  • Ability to work in outreach sites and maintain flexibility in work schedules.

Compensation Package

Our competitive salary range is $85,000 - $115,000 annually, depending on experience and qualifications. We also offer a comprehensive benefits package, including health insurance, retirement savings plan, paid time off, and professional development opportunities.
