Salary Information

The estimated annual salary for this position is $68,000-$82,000 based on industry standards and location. Job Description

We are seeking a skilled Licensed Vocational Nurse (LVN) to join our team as a Care Coordinator for Adults with Disabilities. In this role, you will provide high-quality nursing care and support to individuals with disabilities in our Adult Day Program (ADP). You will be an integral part of our team, working collaboratively with a multi-disciplinary team to ensure the well-being of people supported.

As a LVN at Momentum Innovative Disability Services, your responsibilities will include:

  • Providing skilled nursing care to individuals with disabilities in an ADP setting
  • Implementing and overseeing the healthcare plans for people supported
  • Administering medications and performing medical procedures as needed
  • Collaborating with a multi-disciplinary team to ensure the well-being of people supported
  • Maintaining accurate medical records and documentation
  • Offering support and guidance to Direct Support Professionals (DSPs) and other ADP team members

To be successful in this role, you will need to possess:

  • A valid License Vocational Nurse (LVN) license in the state of California
  • CPR certification
  • Strong communication and interpersonal skills
  • A genuine passion for providing quality care and support to individuals with disabilities
  • Ability to work effectively in a team-oriented environment
  • Willingness to adapt to the needs of the people supported and facility
