About the Role

We are seeking a highly skilled Foster Care Placement Specialist to join our team at Health Connect America. As a key member of our team, you will be responsible for matching children with the most suitable foster care placements, ensuring their unique needs are met.

Responsibilities
  • Develop and maintain in-depth knowledge of available resources and the child welfare system.
  • Collaborate with Child Protective Services and case management organizations to complete intake screenings for new children or those requiring replacement.
  • Gather comprehensive information about each child, including medical, dental, behavioral, educational, mental health, emotional health, trauma history, likes, dislikes, strengths, and needs.
  • Document all efforts in matching children to the most appropriate homes.
  • Ensure all factors are considered when deciding the most suitable placement, including siblings, child's home school, removal area, individual needs of the child, etc.
  • Complete match reports within 24 hours of placement.
  • Work in partnership with our licensing team, agencies, and residential providers to secure the most suitable placement for each child in care.
Qualifications
  • Minimum of a Bachelor's degree in Social Work or a closely related field.
  • At least one year of experience working with special needs children in a child welfare setting.
  • Relevant case management experience or experience with foster care is preferred.
  • Child Welfare Certification is preferred.
