**Job Summary:**
We're seeking a Senior Patient Care Manager to lead our team at Heartland Hospice. As a key member of our organization, you will oversee the daily operations of your assigned hospice site, ensuring the delivery of high-quality patient care. Your responsibilities will include supervising and managing the performance of assigned team members, assigning admit and evaluation visits, and scheduling visits to ensure timely completion.
You will also be responsible for managing the site in the absence of the Executive Director or Administrator, overseeing clinical and administrative functions, and responding to phone referrals/inquiries in a timely and professional manner.
**About You:**
To succeed in this role, you will need:
- A degree in nursing (ASN or BSN) from an accredited school and one year of experience as an RN, licensed in the state of employment
- Three years' experience in direct patient care and one year of management experience, with two or more years in hospice or home care
- The ability to effectively operate nursing assessment tools and office equipment, including computers and tablets
- Sensitivity to the feelings of terminally ill patients and their families before and after death
- An understanding of the hospice concept and its applications
**Licenses, Certifications, and Registration:**
- Current license to practice nursing in the state of employment
- CPR Certification
- Valid driver's license and current automobile insurance

- Demonstrated flexibility and ability to perform various tasks without losing efficiency or composure
- Effective communication skills with diverse socioeconomic backgrounds
