We are seeking skilled LPNs who share our passion for providing personalized home health care. As a member of our team, you will work one-on-one with patients in their homes, helping them recover from hospital stays and thrive with continuous support.

Job Responsibilities
  • Implement physicians' prescribed plans of care for patients, ensuring timely and effective treatment.
  • Conduct comprehensive patient assessments, coordinate care with healthcare teams, and provide detailed documentation in our system of record.
  • Administer prescribed medication, treatments, and therapies as ordered by physicians.
  • Teach family members about health promotion and disease management, empowering them to make informed decisions about their loved ones' care.
  • Foster open communication with clients, families, staff, and Case Managers regarding client concerns and changes in condition.
Requirements
  • Active, unencumbered license (RN/LPN/LVN) in state of practice.
  • CPR certification from American Red Cross, American Heart Association, or BLS.
  • TB skin test within last 12 months.
  • 6 months prior hands-on nursing experience preferred but not required.
  • Reliable transportation required.
