Job Title: Virtual Care Center Registered Nurse

We are seeking a skilled and compassionate Virtual Care Center Registered Nurse to join our team at Trinity Health At Home. As a Virtual Care Center RN, you will provide one-to-one care to patients in their homes, utilizing cutting-edge technology and clinical knowledge to deliver exceptional care.

Responsibilities:
  • Provide client assessment, care planning, intervention, and overall supervision of patient care and outcomes.
  • Follow physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements, and our policies and procedures.
  • Utilize advanced remote monitoring technology and 24/7 access to our Virtual Care Center RNs to enhance clinical excellence.
Requirements:
  • Graduate of an approved nursing education program.
  • One (1) year experience as a professional acute care nurse.
  • Home care experience preferred but not required.
