Nursing Care Coordinator Job Description

At Oklahoma Mental Health Council dba Red Rock Behavioral Health Services, we are seeking a skilled Nursing Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality nursing care and coordinating medical/psychiatric services for our clients. Your primary goal will be to ensure continuity of care and provide exceptional support to our clients and their families.

Key Responsibilities:

Perform comprehensive nursing assessments to identify client needs and develop individualized care plans
Provide treatment for existing and emergency health care conditions, including administering medication and maintaining clinical charts
Assess medical appropriateness for admission and continually evaluate client progress throughout their stay
Collaborate with our psychiatric team to ensure seamless care coordination and referrals to more appropriate levels of care when necessary
Monitor client activities and job duties of Mental Health Assistants/Technicians assigned to their shifts
Review and monitor client progress through supervision and disposition of cases, conferring with other programs/agencies, doctors, etc. as required regarding client welfare
Document all care provided according to nursing standards of care, agency policy, and procedures

Requirements:

Licensed as a Registered Nurse in the state of Oklahoma
Demonstrated interpersonal and communication skills
Experience in use of physical assessment skills and ability to handle medical emergencies
Ability to work shift-work and one year of experience preferred
Knowledge and experience working with administration of psychotropic medications, preferred
Knowledge of psychiatric nursing, practices, techniques, and procedures, preferred
Experience working in a psychiatric setting, preferred
Certification in mental health nursing a plus
