About the Job
BAYADA Home Health Care is seeking a compassionate Home Health Physical Therapist to join our team. This is a full-time, pay per point position.

Responsibilities:
As a Jefferson Health at Home by BAYADA Physical Therapist, you will use your clinical skills and compassionate heart to treat patients where they want to be – in the comfort of their own home. You will see a client through their care needs, from hospital discharge to living a safe home life with comfort, independence, and dignity. You will perform diagnostic tests and measurements, develop and implement individualized care plans, and continually assess and revise the Physical Therapy - PT care plan.

Requirements:
Active and current license to practice as a professional Physical Therapist (PT) in the state of New Jersey. Previous home healthcare experience is welcomed but not required. Ability to travel to cases as assigned, and a calm and reassuring presence with excellent assessment skills and strong written and verbal communication are essential.
