We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team in the outpatient clinic. As a key member of our healthcare team, you will provide high-quality patient-centered care while working collaboratively with physicians and other healthcare professionals.

Job Summary

This full-time position is responsible for providing medical care to patients in the outpatient setting. You will work closely with our multidisciplinary team to develop and implement individualized plans of care that promote optimal health outcomes.

About the Role

This role involves:

  • Providing direct patient care, including assessment, diagnosis, treatment, and management of acute and chronic conditions.
  • Developing and implementing individualized plans of care in collaboration with other healthcare professionals.
  • Conducting thorough assessments of patients' physical and emotional needs, identifying areas for improvement, and developing strategies for addressing these needs.
  • Collaborating with healthcare teams to ensure seamless transitions between care settings and stages of treatment.
Requirements

To be successful in this role, you will need:

  • A degree from an accredited school of nursing (BSN or equivalent) and current licensure as a Registered Nurse in the State of Vermont.
  • BLS Certification through the American Heart Association and ACLS, TNCC, PALS, ENPC, and CPI certifications as required by our operational framework.
  • At least 2 years of prior nursing experience, preferably in an office/clinic practice setting.
  • Demonstrated proficiency in acute-care nursing, knowledge, and skills.
Compensation and Benefits
