Registered Nurse (RN) - Home Health Wound Care

BAYADA Home Health Care, a leading provider of home health care services, is seeking a skilled Registered Nurse (RN) to join our team as a Home Health Wound Care specialist in Souderton, Pennsylvania.

About the Role

This is a unique opportunity for a compassionate and detail-oriented RN to make a meaningful impact on patients' lives by providing specialized wound care services in the comfort of their own homes.

The ideal candidate will have a strong background in nursing, excellent assessment skills, and the ability to deliver patient-centered care with empathy and professionalism.

Key Responsibilities
  • Conduct comprehensive assessments of patients' wounds and develop individualized care plans to promote healing and prevent further complications.
  • Administer medications, provide wound care treatments, and perform other necessary interventions to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to coordinate care and address concerns.
  • Maintain accurate and detailed records of patient interactions and treatment progress.
Requirements
  • Active and current license to practice as a professional registered nurse (RN) in the state of PA.
  • Wound, Ostomy and Continence Nurse (WOCN) certification required.
  • At least one year of nursing experience, preferably in a home health or wound care setting.
  • Prior experience with electronic medical records (EMRs) and computer-based documentation systems.
