Job Title: Diabetes Care Specialist

Job Summary:

We are seeking a highly skilled Diabetes Care Specialist to join our team. The successful candidate will work closely with physicians and advanced practice providers to ensure integrated and comprehensive management of patients with diabetes-related disorders.

Key Responsibilities:

- Provide comprehensive diabetes management and care coordination
- Conduct assessments and develop individualized care plans
- Deliver counseling and therapeutic interventions to patients
- Collaborate with an interdisciplinary team to ensure integrated care

Requirements:

- Registered Nurse (RN) or Registered Dietitian (RD) degree
- Licensed Diabetes Educator certification
- Ability to work effectively with a wide range of constituencies

Preferred Qualifications:

- CDCES or BC-ADM certification
