About the Role

We are seeking a highly motivated and compassionate Community Support Specialist to join our team in the Supported Housing Program. This individual will play a critical role in providing support services to individuals enrolled in our program, helping them achieve stability and independence.

Key Responsibilities
  • Schedule and complete intakes for potential program participants, ensuring a smooth onboarding process.
  • Complete all required Supported Housing paperwork and reports, including data inputting and monitoring in AWARDS, to maintain accurate records.
  • Conduct regular housing inspections/visits and annual apartment inspections with program participants, promoting a safe and healthy living environment.
  • Provide case management and support services to all program participants, addressing their unique needs and challenges.
  • Develop and maintain positive relationships with landlords, communicating effectively to ensure seamless service delivery.
  • Represent MHA at Single Point of Access (SPOA) as needed, advocating for program participants' needs.
  • Attend monthly staff meetings and training sessions, staying up-to-date on best practices and program developments.
Requirements

To be successful in this role, you will need:

  • A high school diploma or equivalent.
  • At least 1 year of experience in a related field, preferably in mental health or social services.
  • The ability to work independently with minimal direct supervision, demonstrating self-motivation and organizational skills.
  • Effective written and oral communication skills, enabling you to communicate complex information clearly.
  • Organizational, self-motivated, and able to coordinate multiple tasks simultaneously, prioritizing tasks efficiently.
  • Ability to exercise sound judgment under crisis situations, maintaining confidentiality and adhering to regulations.
  • A valid NYS Driver's License, access to a vehicle, and a clean driving record acceptable for agency insurance coverage.
  • Clearance under the Medicaid Fraud & Abuse process, ensuring compliance with regulatory requirements.

Compensation Details
