Midwest Orthopaedics at Rush is seeking a skilled Physical Therapist to provide high-quality outpatient physical therapy services to patients in the comfort of their own homes. As a Home-Based Physical Therapist, you will work independently to devise and execute detailed treatment plans for post-operative patients, maintaining friendly and cordial relations with clients and employees alike.
Key Responsibilities
- Provide one-on-one in-home therapy for post-operative patients
- Develop and implement treatment plans tailored to individual patient needs
- Maintain accurate patient charts, clinical logs, and checklists for compliance
- Establish and monitor home exercise programs
- Provide education on diagnosis and symptom management
- Respond to patient care priorities with initiative and sound judgment
Requirements
- Bachelor's Degree from an accredited Physical Therapy Program
- Minimum one-year experience in outpatient setting preferred
- Current State of Illinois Physical Therapy license
- Current CPR Certification
- BCLS or ACLS
- Valid Driver's License, reliable transportation, and required liability insurance
