Urrly is seeking a highly skilled Nurse Practitioner to lead a groundbreaking care model transforming the lives of nursing home residents.



  • Deliver Exceptional Primary Care to a dedicated panel of patients, focusing on their unique health goals.
  • Champion the 'Treat in Place' Model, ensuring advanced care planning is seamless and tailored to each individual.
  • Conduct Thorough Assessments, create personalized care plans, and coordinate with care teams to ensure the best possible outcomes.
  • Play a Key Role in Clinical Operations such as prior authorizations, medication reviews, and closing quality care gaps (HEDIS and STARs).
  • Act as a Trusted Advisor to patients, families, and care teams, ensuring everyone is informed and aligned on care needs and goals.


  • Unrestricted State Licensure as an Advanced Practice Nurse and national board certification.
  • Eligible for Medicare Participation.
  • 6+ Months of Experience as an Advanced Practice Provider (preferably with geriatric, hospice, or palliative care populations).
  • Strong Clinical Skills and experience in treating chronically ill patients, with a preference for nursing home environments.
  • Tech-Savvy with Excellent Communication Skills to collaborate effectively with patients and healthcare teams.
