Job Summary

The Community Health Worker (CHW) plays a vital role in enhancing patient care by fostering strong relationships between patients, healthcare teams, and community resources. Key Responsibilities
  • Conduct home visits and phone contacts to engage patients in their care plans
  • Collaborate with healthcare teams to ensure seamless communication and coordination of care
  • Provide education and support to patients and families on health-related topics and resources
  • Facilitate referrals to community resources and services to support patient needs
  • Monitor patient progress and outcomes, making adjustments to care plans as needed
Requirements
  • High School Diploma or Equivalent (Required)
  • Completion of 75 Hours Community Health Worker Core Training (Required)
  • General Experience and knowledge of protocols with Electronic Medical Records (EPIC) (Preferred)
  • General Experience in medical or health-related field and knowledge of medical terminology (Preferred)
