About the Role

The RN On Call (RNOC) is a critical role at Sage Hospice LLC, where you will plan, organize, and direct hospice care with a focus on serving patients while on call. As a professional nurse, you will build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Responsibilities:

  • ">
  • Promote and practice the Sage Family of Companies philosophy and mission, providing compassionate, comprehensive, and qualified care to people with a life-limiting illness.">
  • Follow the initial, comprehensive, and ongoing assessment of patient and family established by the Registered Nurse Case Manager (RNCM) to determine hospice needs.">
  • Provide professional nursing care by utilizing all elements of the nursing process.">
  • Evaluate patient's status by following the plan of care and notifying the Interdisciplinary Group (IDG) of needed updates due to after-hour on-call interventions.">
  • Initiate appropriate preventative nursing procedures and on-call medications and treatments as prescribed by the physician in the physician's plan of care.">
  • Counsel and provide education to the patient and family/caregiver during periods of crisis to facilitate decision-making and allow the patient to remain in the care setting of their choice.">
  • Provide healthcare instructions to the patient as appropriate per assessment and plan.">
  • Assist the patient with activities of daily living and facilitate the patient's efforts toward self-sufficiency and optimal comfort care.">
  • Complete, maintain, and submit accurate and relevant clinical notes regarding patient's condition and care provided.">
  • Communicate with the physician regarding the patient's needs and report changes in the patient condition; obtain, receive, and document physician's orders as required.">
  • Educate the patient and family/caregiver on self-care techniques and other tasks as they relate to the patient's plan of care.">
  • Provide medication, diet, and other instructions as ordered by the physicians.">
  • Recognize and utilize opportunities for health counseling with patients and families/caregivers.">
  • Work in concert with the interdisciplinary group (IDG).">
  • Provide and maintain a safe environment for the patient.">
  • Assist the patient, family, or caregiver and other team members in providing continuity of care.">
  • Work in cooperation with the family/caregiver and hospice IDG to meet the emotional needs of the patient and family or caregiver.">
  • Ensure that arrangements for equipment and other necessary items and services are available during on-call visits.">
  • Assume responsibility for personal growth and development, maintaining and upgrading professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.">
  • Fulfill the obligation of requested phone triage registered nurse.">
  • Perform other duties as assigned.">

Requirements:

  • Registered Nurse of an accredited school of nursing with a current license to practice professional nursing in the state in which we do business.">
  • One (1) year of acute care experience in long-term care, hospice, or home health setting.">
  • Must have reliable transportation, proof of current automobile insurance, and a valid driver's license.">

Salary: The estimated salary for this role is $80,000 - $100,000 per year, depending on location and experience.
