Outpatient Therapist

**Job Summary:**
We are seeking a highly skilled Outpatient Therapist to join our team at Axesspointe Community Health Center.
About the Role:
The successful candidate will be responsible for providing high-quality direct client care services and community support programs to clients with severe mental disabilities.
Key Responsibilities:
- Evaluate client needs for services, assess appropriateness of requests, and facilitate referrals as needed.
- Conduct diagnostic assessments to determine medical necessity and develop individual service plans.
- Provide psychotherapy/counseling services through individual, group, and family methods.
- Review and update ISPs, progress, and diagnoses on a scheduled basis.
- Enter and maintain daily schedules in computer systems.
- Achieve and maintain productivity standards and provide crisis intervention services as needed.
- Make referrals to other services within the community and agency as indicated.
- Participate in staff meetings, training, supervision, and committees to stay current on procedures and policies.
- Consult, collaborate, and communicate with clients, staff, and external agencies to ensure coordination of care.
- Serve other departments, programs, and service locations as needed and assigned.
- Complete documentation in a timely manner according to VCS policies and provide necessary data for statistical and billing purposes.
Requirements:
- Master's Degree in Social Work or Counseling from an accredited university.
- Licensed as an LPC/LPCC or LSW/LISW or have completed coursework and passed examinations for LPC.
- Knowledge of federal, state, and local laws, Ohio Department of Mental Health standards, and professional ethical standards.
- Ability to lift up to 25 lbs.
