Job Summary

The Community/Mobile Integrated Health (CP/MIH) Paramedic functions within Covenant Health's Mobile-Integrated Health Programs and the Covenant EMS System. This role involves acting as a member of the MIH team for Hospital-at-Home patients, providing remote monitoring technology, conducting physical assessments, and collaborating with an RN and provider team to manage patient care.

Key Responsibilities

Works hand-in-hand with a virtual RN to manage the care of patients in their homes as part of the Hospital-at-Home program.

Communicates with other members of the interdisciplinary team, including other MIH team members, management, and outside agencies.

Collaborates seamlessly with the network of partners that make this possible, including patients, their families, public safety agencies, training institutions, hospital personnel, and others as needed.

Ensures patients' encounters documentation in Electronic Health Record is accurate and timely, in addition to any other required documentation.

Manages all aspects of CP/MIH vehicles and clinical equipment, including cleaning and maintaining vehicles and facilities daily, managing supply inventory, and ensuring the team has adequate supplies on-hand to successfully conduct operations.

Facilitates scheduling needs, equipment and supply maintenance, and other daily operational support functions as assigned by the CP/MIH Manager.

Supports other programs and divisions within the Covenant EMS System as able, and remains up-to-date on all regulatory training and ensures that all certifications are uploaded into the appropriate systems.

Prepares reports and manages projects as assigned by the CP/MIH Manager, and works with the CP/MIH Manager and/or EMS Medical Director to ensure all lab compliance rules are adhered to.

Coordinates with the CP/MIH Manager, the EMS Director, and the Covenant Health IT Team to facilitate and manage all aspects of IT, including computer maintenance and systems account setup for team members.

Requirements

Must have and maintain a TN Critical Care Paramedic certification and International Board of Specialty Certification (IBSC) Community Paramedic Certification.

Must have a valid driver's license and state mandated minimum insurance coverage, and meet Covenant Health's minimum standards for driving records at the date of hire and throughout employment tenure.
