Nurse Practitioner for In-Home Visits

We are seeking board-certified nurse practitioners to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. As an in-home nurse practitioner, you will have the opportunity to work with patients in their own homes, providing personalized care and attention.
Key Responsibilities:
- Conduct in-home visits to assess patients' overall health and well-being
- Identify potential barriers to care and develop personalized plans to address them
- Work with patients to develop and implement care plans that meet their unique needs

Requirements:
- Master's degree in nursing
- Current North Carolina nurse practitioner license
