Home Health Therapist Specialist

BAYADA Home Health Care is seeking a highly skilled and compassionate Home Health Therapist Specialist to join our team in Woodbridge, New Jersey.
About the RoleThis is a full-time opportunity to make a meaningful difference in the lives of adult and senior clients by providing personalized home health care services. As a Physical Therapist, you will work independently to assess clients' needs, develop and implement effective care plans, and educate clients and their families on rehabilitative care techniques.
Responsibilities- Conduct thorough assessments of clients' physical abilities and develop personalized care plans to improve mobility, strength, and overall well-being.
- Perform diagnostic tests and measurements, such as range of motion, transfer status, balance testing, and safety assessments.
- Implement evidence-based interventions, including manual therapeutic exercises, gait training, and other activities to promote wound repair and prevent further injury.
- Educate clients and their families on proper care techniques, such as wound management, ambulation, and transfer strategies.
- Maintain accurate records of client interactions, interventions, and progress using state-of-the-art technology.
- A current NJ Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
