Role Summary

As a member of our multidisciplinary team, you will play a vital role in delivering compassionate end-of-life care to patients and their families. Your expertise will help ensure a peaceful and dignified transition, fostering trust and understanding in a time of need.

Key Responsibilities

• Consult with the team to evaluate patient status and appropriateness of care, including admission, length of stay, transfer, and discharge.

• Provide direct treatment and management of health conditions, making recommendations to providers regarding additional services needed and referrals for community resources.

• Develop and implement patient care plans to optimize outcomes and maximize available resources.

• Participate in interdisciplinary team planning to collaborate with other disciplines to determine the appropriate care and services needed.

Qualifications

A current license as a Registered Nurse in Maine or a compact state is required. New graduate RNs are encouraged to apply. Valid state driver's license with an acceptable driving record is also necessary.
