Patient Care Specialist

We are seeking a dedicated Patient Care Specialist to join our team at ResCare Community Living. In this role, you will be responsible for providing top-notch patient care services to individuals in need of assistance with daily living due to an intellectual, developmental or cognitive disability.

As a Patient Care Specialist, your primary responsibility will be to provide 1:1 care for clients, following their individualized plans of care while maintaining their dignity and independence. This includes monitoring vital statistics, reporting any abnormalities to the case manager and physician, and ensuring infection control policies are upheld at all times.
You will also be responsible for administering medications, feedings, oxygen, and other treatments as needed, according to each client's plan of care. Additionally, you will maintain accurate records of nursing actions, communicate with family members, case managers, and physicians, and document progress and outcomes for established goals.
RequirementsTo be considered for this role, you must have completed post-secondary coursework leading to licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). You should possess knowledge of individual service plans, including understanding of client rights and health needs.
A valid LPN or LVN license in good standing from the state(s) of practice is also required. CPR certification is highly preferred. Excellent interpersonal skills, patience, empathy, and compassion are essential for success in this position.
In-depth understanding of patient care policies and regulations, as well as familiarity with symptoms and signs of improvement for various medical conditions, are desirable qualifications.
