Effective collaboration with fellow professionals, caregivers, and patients/families is paramount for seamless care coordination and the development of appropriate goals and discharge plans.

Key Responsibilities:

* Conduct evaluations and assessments to identify client needs and develop individualized treatment plans
* Execute interventions and monitor progress towards set goals, adjusting treatment plans as needed
* Collaborate with fellow professionals, caregivers, and patients/families to ensure seamless care coordination and develop appropriate goals and discharge plans
* Maintain accurate and detailed records of client progress and treatment plans

Requirements:

* Current state licensure as a Physical Therapist
* Experience in patient care and home care
* Proficiency in appointment management, documentation, and record-keeping
* Exceptional interpersonal and communication skills
* Strong analytical and problem-solving abilities
* Ability to thrive both independently and as part of a team
* Knowledge of developmental disabilities and associated interventions
* Experience in pediatric physical therapy
* Article 9 training
* CPR Certification
* Current AHCCCS ID or willingness to obtain one
