Ambulatory Care Nurse

Atlantic Health System is seeking a skilled Ambulatory Care Nurse to join our team. As an Ambulatory Care Nurse, you will be responsible for providing high-quality patient care in an outpatient setting. Your primary focus will be on promoting health, preventing illness, and restoring health to patients in a variety of ambulatory care settings.
Key Responsibilities:
- Provide direct patient care, including assessing patient needs, developing and implementing individualized care plans, and evaluating patient outcomes.
- Collaborate with healthcare teams to coordinate patient care and ensure seamless transitions between care settings.
- Conduct patient education and health promotion activities to empower patients to take an active role in their care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Document patient information accurately and thoroughly in electronic medical records.
Requirements:
- BSN required.
- Acute care experience.
- BLS, ACLS required.
- Specialty knowledge, skills, and abilities may be required based on department/specialty group assigned.
- Prior Ambulatory Surgery OR Medical/Surgical experience is required.
