Job Description
Our Mental Health Advocate Specialists play a vital role in helping individuals and families navigate the mental health system. Your primary responsibility will be to provide case management services, linking clients to necessary resources and ensuring they receive the support they need to thrive in their communities.
Key Responsibilities:
- Conduct comprehensive assessments to identify client needs and develop personalized service plans
- Link clients to community resources, including healthcare services, housing, and employment opportunities
- Provide ongoing support and advocacy to clients, empowering them to make informed decisions about their care
- Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination
- Document client progress and service delivery in accordance with regulatory requirements

Requirements
To be successful in this role, you will need to possess a bachelor's degree in a related field, such as sociology, psychology, or social work.
