Salary and Benefits

We offer competitive compensation, medical benefits, retirement plans, wellness programs, and ongoing learning opportunities to grow your career. The base pay range for this role is between $100.00 and $130.00 per assessment at the commencement of your first year of employment.

About the Role

We are seeking a compassionate and experienced RN to join our Licensed Home Care team. As a Nursing Professional for Home Care, you will play a vital role in assessing our patients' current needs and developing personalized care plans.

Your responsibilities will include:

  • Providing care and services according to scope and standards of nursing practice and adhering to the Code of Ethics for Nurses
  • Respecting client and employee rights and privacy, ensuring the security of protected information, and practicing in an ethical manner
  • Following agency policies and procedures regarding infection control measures
  • Assessing and regularly reassessing client needs and responses to care, treatment, and education
  • Developing, implementing, evaluating, revising, and providing input to client plans of care
  • Completing all client documentation, including written records, reports, and notes
  • Actively coordinating care through communication and written documentation
  • Supervising ancillary staff involved in client plans of care
  • Providing education to clients and families through assessment, development of teaching objectives, and implementation of teaching plans
  • Interpreting the philosophy and objectives of the agency to clients, families, and others
  • Participating in the agency's Performance Improvement Program
Requirements and Qualifications

To be successful in this role, you will need:

  • At least an Associate's Degree and current NYS License in good standing
  • 3+ years of nursing experience
  • Ability to travel within the 2 boroughs
  • Current knowledge of state and federal home health regulations and community service systems
  • Proficiency in Microsoft (Word, Excel, Outlook)
  • Excellent verbal and written communication, organizational, and time management skills
Bilingual Ability and Nice-to-Haves

We welcome bilingual candidates who speak English and one of the following languages: Spanish, Russian, Chinese, Korean, or Bengali. Additionally, experience in Home Care, Geriatrics, and Case Management; field experience assessing patient care is highly desirable.
