Patient Care Coordinator

In this role, you will collaborate with healthcare professionals to evaluate patients' medical conditions and assess client needs as part of an interdisciplinary team. Our ideal candidate is a detail-oriented individual with excellent communication skills who can work effectively in a fast-paced environment.
About the RoleThe Clinical Social Worker I will be responsible for developing and implementing treatment plans that support patient-centered care. This includes collaborating with healthcare team members to monitor, evaluate, and record client progress according to measurable goals described in treatment and care plans.
You will also demonstrate fundamental skills in formulation, assessment of risks, crisis intervention, completion of psychosocial treatment plans, and appropriate documentation. Additionally, you may provide assistance and support with onboarding for new team members and help promote collaboration and equity among health care team members, other colleagues, and organization to support and enhance patient care.

We are looking for a qualified candidate with a Bachelor's Degree in Social Work or a related field. A Master's Degree in Mental Health & Behavioral Medicine is preferred. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
