### Job Overview
TotalMed Staffing is dedicated to providing exceptional healthcare services. As a Nurse Specialist in our Transition Care Unit, you will play a vital role in delivering specialized care to patients transitioning from critical care to general medical-surgical units. This position requires a unique blend of clinical expertise, effective communication skills, and adaptability in a fast-paced environment.

### Estimated Salary
$1,800.88 per week

### Job Responsibilities

#### Key Responsibilities
• Provide specialized nursing care to patients transitioning from intensive care to general medical-surgical units, ensuring seamless continuity of care.
• Monitor and assess patients' vital signs, cardiac rhythms, and respiratory status in the transition care unit, making informed decisions to optimize patient outcomes.
• Administer medications, perform procedures, and collaborate with interdisciplinary teams to ensure comprehensive patient care, promoting a culture of safety and quality.
• Educate patients and families on post-intensive care recovery, medication management, and self-care techniques, empowering them to take an active role in their healthcare.
• Communicate effectively with healthcare team members to facilitate patient progress and optimize outcomes in the transition care unit, fostering a collaborative and dynamic environment.

#### Additional Responsibilities
• Collaborate with the healthcare team to develop and implement individualized care plans, addressing the unique needs of each patient.
• Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of evidence-based practices.
• Stay up-to-date with the latest clinical guidelines, research, and best practices in transition care, applying this knowledge to improve patient outcomes.

### Required Qualifications
• Current RN license
• BLS certification (other certifications as required by the facility)
• Two years of recent experience in a transition care setting
• Strong communication and adaptability skills, with the ability to work effectively in a fast-paced environment
