Nurse Care Coordinator for North Carolina Communities

We are seeking a skilled and compassionate Nurse Care Coordinator to join our team at UNC Health. In this role, you will be responsible for delivering high-quality patient care while utilizing the nursing process in a hospital setting.
Responsibilities:
- Assess patient needs and develop individualized plans of care that promote optimal outcomes.
- Collaborate with interdisciplinary teams to ensure seamless coordination of care.
- Provide education to patients and families on disease management, treatment options, and self-care techniques.
- Evaluate patient progress and adjust care plans as needed.
- Participate in quality improvement initiatives to enhance patient satisfaction and clinical outcomes.
Requirements:
- Degree in Nursing from an accredited institution.
- Licensed to practice as a Registered Nurse in the state of North Carolina.
- BLS certification for Healthcare Providers.
- At least one year of experience in a hospital setting.
