We are seeking a dedicated and experienced Inpatient Advanced Practice Healthcare Provider to join our innovative Hospital at Home program. This dynamic role offers an exceptional opportunity to deliver high-quality inpatient-level care in the comfort of patients' homes.

Key Responsibilities:

  • Conduct comprehensive evaluations and assessments of acutely hospitalized patients, utilizing strong clinical acumen to interpret and respond to complex cases effectively.
  • Collaborate with attending physicians and Command Center staff to develop and implement individualized inpatient care plans that meet the unique needs of each patient.
  • Provide exceptional patient care in diverse settings, including hospitals, virtual platforms, and patients' homes, leveraging excellent verbal and written communication skills to deliver compassionate, patient-centered care.

About the Role:

  • This full-time, day shift position requires a minimum of 2 years of experience in inpatient advanced practice roles, with a strong focus on adaptability and comfort working in a dynamic and evolving environment.
  • A competitive salary is offered, estimated to be around $120,000 per year, based on industry standards and the location's cost of living.
  • In addition to a comprehensive benefits package, which includes starting benefits on Day 1 of employment, you will also receive $2,000 annually for continuing medical education and a generous time-off package.
