You will work closely with participants to assess their needs, develop individualized service plans, and connect them with the appropriate resources and support systems. Your expertise in case management will enable you to provide intensive individual mental health interventions and support services.
To be successful in this role, you will need a Master's degree in social work, psychology, or a related field, as well as experience working in a community-based agency program.
