Master's Caseworker

We are seeking a highly skilled Master's Caseworker to join our team at FRESENIUS MEDICAL CENTER. As a Clinical Social Worker, you will provide essential psychosocial services to patients undergoing dialysis, ensuring their optimal quality of life and rehabilitation.

• Participate in care planning with patients and healthcare teams to identify effective interventions for rehabilitation and treatment goals.
• Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
• Offer supportive counseling services to patients, adhering to clinical training and state licensure.
• Collaborate with the interdisciplinary team to promote positive adjustment, rehabilitation, and improved quality of life for patients.
• Document interactions and interventions provided to patients and/or families, adhering to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy.

• Masters in Social Work degree
• Knowledge of Social Work Theory of Human Behavior and accepted methods of social work practice
• Ability to work with diverse patient populations and healthcare teams
• Excellent communication and interpersonal skills
