Cleveland Clinic's Heart, Vascular and Thoracic Institute is seeking an experienced Advanced Practice Provider to join our Heart Transplant team on a part-time basis.

The ideal candidate will be a motivated, team-oriented individual with excellent verbal and written communication skills, and a passion for providing exceptional patient care.

This provider will work in collaboration with a physician to triage patients, practice in both inpatient and outpatient settings, and adhere to UNOS and CMS regulations.

Schedule:

20 hours a week

Responsibilities:


Conduct thorough medical histories, perform complete physical examinations, initiate lab and radiology tests, and scrutinize lab data to establish a patient's current health status and develop a working diagnosis and treatment plan.

Perform and interpret common laboratory, radiologic, and diagnostic procedures, and take initiative in performing evaluation and therapeutic procedures in response to life-threatening situations.

May prescribe and monitor medications, and perform specialized procedures such as bone marrow aspirations, biopsies, and harvest, blood draws, and designated IV starts.

Provide information and answer routine questions of patients regarding their disease, and participate in medical team rounds and collaborate with nursing and other healthcare team members to ensure quality patient care.

Refer patients to specialists as appropriate, and ensure continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team.

Assist in data analysis, and participate in quality monitoring through the review of records and treatment plans for patient outcomes.

Facilitate appropriate length of stay, discharge planning, and compliance with regulatory standards for inpatient management.

May serve as clinical preceptor for PA/APRN students and other health professionals.

Graduate of an accredited APRN program, and currently licensed as a Nurse Practitioner in applicable state(s).

National certification as a Nurse Practitioner by nationally accredited organization recognized by state Board of Nursing.

Basic Life Support (BLS) through the American Heart Association (AHA) required.

Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) through the American Heart Association (AHA); or Neonatal Resuscitation Program (NRP) through the American Academy of Pediatrics (AAP) may be required based upon work setting.

Must have arrangements with at least one physician in accordance with requirements set forth by the State Board of Nursing in applicable states.

Must be credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.

Certification as a Physician Assistant through the NCCPA or successor organization recognized by their practicing states Licensing Board.

Basic Life Support (BLS) through the American Heart Association (AHA) required.

Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) through the American Heart Association (AHA); or Neonatal Resuscitation Program (NRP) through the American Academy of Pediatrics (AAP); or Advanced Trauma Life Support (ATLS) American Heart Association may be required based upon work setting.

Must have a Physician Supervision Agreement and signed addendum to Physician Supervision Agreement.

Must be credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.

Related clinical experience preferred.

Follows standard precautions using personal protective equipment as required.
