Nurse Care Manager

About the Role

We are seeking an experienced LPN or LVN to join our team. As a Nurse Care Manager, you will be responsible for administering medications and treatments in a safe and organized manner. You will also conduct Service and Health Updates, participate in the development of Individualized Service Plans, and maintain accurate documentation.

Responsibilities
  • Medication Administration: Provide high-quality care by administering medications and treatments safely and efficiently.
  • Service and Health Updates: Conduct daily updates on resident health and services.
  • Individualized Service Plans: Participate in the development and implementation of personalized care plans.
  • Documentation: Maintain accurate and up-to-date records of resident care and services.
Requirements
  • LPN or LVN licensure.
  • CPR and First Aid certification.
  • Previous experience working with seniors preferred.
